As a non profit organization, and along with attempting to make this affordable for every family, we are looking for sponsors. If you are, know, or work for a business that may be interested in sponsoring Crusader Jr. Football; please click to fill out this form. We will then get sponsorship information out to those businesses submitted.
Our Program needs to ensure parents and players understand there are responsibilities to others while being a team member. This form needs to be filled out, signed, and returned to Crusader Jr. Football prior to the Player Participating.

Crusader Jr. Football uses the Kansas State High School Athletic Association (KSHSAA) Physical Form.
Younger players can use the Kansas Child Health Assessment Form in place of the KSHSAA Physical Form if their doctor prefers.
Crusader Jr. Football uses the Kansas State High School Athletic Association (KSHSAA) Concussion & Head Injury Information Release Form.
